Pop Warner Registration Today

When coming to registration you must also have the following:
1 copy of your medical insurance card
1 copy of the child’s birth certificate
1 passport style picture of the child participating
2 copies of the child’s report card by the first week of practice together with the Doctor’s Medical form.

YOUR CHILD MUST BE PRESENT FOR WEIGH IN (EXCEPTION OF FLAG)

All payments must be made at the time of registration.
REGISTRATION COSTS (Note FLAG discount)
1 child $100.00 (FLAG $50.00*) + $100.00 for raffle tickets
2 children $150.00 (FLAG $100*) + $150.00 for raffle tickets
3 children $200 (FLAG $150*) + $150.00 for raffle tickets
* all children must be at flag level ONLY for discount

REGISTRATION INCENTIVE!!!! ANY RETURNING PLAYER OR CHEERLEADER WHO BRINGS A FRIEND WILL GET $50.00 OFF THEIR REGISTRATION FEE. FRIEND MUST BE A BRAND NEW PARTICIPANT AND NOT PREVIOUSLY INVOLVED IN CLARK POP WARNER!!!!

No registration forms will be accepted unless ALL of the proper paperwork and forms are completed at the time of registration and payment is present. NO COPIES WILL BE MADE AT REGISTRATION.
